WHAT?

Through Supported Employment, staff from our agency assists eligible participants match their interests and skills to jobs. 

Supported Employment puts eligible participants including persons with developmental disabilities to work in community business like yours. 
HOW?​

  • Assist employers with orientation and training of the employee
  • Consult with employers and staff to enhance the understanding of, and interaction with, the new employee
  • Provide the ongoing training and support that may be needed by the employer and/or employee to assure successful continued employment

Community Access Services

CCTC's Community Access Services are designed to assist the participant in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active community participation and independent functioning outside the participant’s home or family home. These services may include:
​
  • mobility training
  • training in the areas of daily living skills ​ (including leisure/recreation skills)
  • communication training
  • programming to reduce inappropriate and/or mal-adaptive behaviors
  • training in the use of common community resources

     The emphasis of training is on assisting the individual in increasing self help, socialization skills, skills for daily living, and adaptive skills required for active community participation and independent functioning outside the participant’s home or family home. These activities include accompanying the individual to the grocery store, or eating establishments; teaching an individual how to participate in social and recreational activities; and assessing other activities of community living.
     Community Access Services are individually planned to meet the participant’s needs and preferences for active community participation.

     These services are provided in either community-based or facility-based settings but not in the participants home or family home or any other residential setting.  The intended outcome of these services is to improve the participant’s access to the community through increased skills and/or less paid supports.
     Many clients choose to participate in volunteer activities i.e. community soup kitchen, museums, recreation department, assisted living and nursing homes, etc.
Community Access services may be provided in a group setting or an individual setting depending on the desires of the participants.

Adult Day Health

Adult Day Health Staff provide daytime support for aging and/or disabled adults who are experiencing significant functional impairments as a result of a variety of disabilities.

The Adult Day Health program supports the health, nutritional, social, and other daily living needs of
adults in a professionally staffed, group setting. Care plans are overseen and approved by our RN and
implemented by our LPN. Each Adult Day Health staff is proxy caregiver certified. The Carroll County
Training Center Adult Day Health Transportation is provided for anyone who qualifies for Non-
Emergency Medicaid Transport Services. Each qualified participant will be transported to and from our
facility.

*The activities reflect each individual’s needs, abilities, and interest (Nursing services, health monitoring,
medication administration).

*The supervision appropriate with the needs of participants, social and leisure activity programming,
individual and group activities that encourage creativity, social interaction, and physical activity
appropriate to each participant, functional status and ability.

*Nutrition program includes complimentary morning and afternoon snack and lunch

Eligibility:

Service Options Using Resources in Community Environments (SOURCE)
Community Care Services Program (CCSP)
Self-Pay Model for individuals who do not qualify for Medicaid Programs

Program Hours: 8:30am-2:30pm
Monday-Friday
